Comparison on Efficacy of Oral Loratadine and Levocetirizine in Chronic Idiopathic Urticaria

Abstract

Introduction: Chronic idiopathic urticaria (CIU) is a condition characterized by pruritus and wheals. Newer treatments are being developed, but antihistamines remain the mainstay of the treatment. Newer generation antihistamines such as loratadine and levocetirizine have already proved to be efficacious for CIU. This study was done with the aim of comparing the efficacy of oral loratadine and levocetirizine in the treatment of CIU in context of Nepalese population.

Methods: This prospective, randomized, open label study was carried out in 70 patients with CIU in the Department of Dermatology, Nepal Medical College and Teaching Hospital (NMCTH), Kathmandu. All patients diagnosed with CIU by dermatologist were clinically assessed and recording of baseline symptom scores was done. Patients were randomized into group A and group B. Group A patients received oral loratadine and group B patients received oral levocetirizine. Reduction in the symptoms after drug therapy were recorded after 4 weeks. The efficacy of two drugs after 4 weeks of treatment was compared on the basis of reduction in total symptom score (TSS).

Results: There was very highly statistically significant reduction (p < 0.001) in mean TSS after treatment as compared to day one in both the drug groups. There was very highly statistically significant difference (p<0.001) in the mean TSS between the two drugs, levocetirizine being better of the two.

Conclusion: Oral levocetirizine 5mg was superior to oral loratidine in decreasing the symptoms of CIU after 4 weeks of treatment
